1) Please print out our Alterations Measurement Form, fill it out completely and enclose with Repair Request Form, payment from the quote and the wetsuit being sent for alterations.

2) Suggestions for measuring:
- Measurements should be taken by someone other than yourself
- Measurements should be taken next to the skin or while wearing a bathing  suit
- Always use a sewing tape measure, never a ruler or hard tape measure
- Pull measuring tape snug but not tight

3) -PLEASE FILL OUT THE CHART COMPLETELY EVEN IF YOU THINK WE DON'T NEED IT
